There are only about 5 spots on the body and fins, very small. I have a large cleaner shrimp but I have never seen him clean any fish.

Cleaner shrimps are good, you might even want to try a cleaner wrasse, they unfortunately in most cases dont live long but they can sometimes do a good job on parasites. All that said I have had one in my 27g for a year now and still going strong....if he is eating and not getting skinny or lathargic you will probably see him pull through.
