Typically you would just add more "s" beside each other. 3 gang = sss. With a line drawn to each load from each "s". As for plugs, same goes. If you want a plug at regular height (example, 12" off floor) and a plug above it, say 48" off floor, then you would draw in 2 plugs immediately next to each other, with a note to one of the plugs that it is to be mounted at 48". If the plug is to be in an exact height, be sure to note the measurement is to the top / bottom of the plug box.
