Very hard to tell from the video but just looks like some kind of tube worm, I wouldnt be to concerned about it. I have them all over my tank.

Barnacle perhaps look at the bottom middle picture on THIS page

Just checked out the picture, the description matches almost 100% to the barnacles, andthe base of creature looks exactly the same to the picture you pointed out. Also the hammer is very suppressed as you can see in the vid...not dying tho, so i'll see if hammer will branch off in other directions.
