I use cured liverock. It is good for any treatment except copper. I never had to treat with copper so far though.

I never had any ammonia problem in my QT when having liverock in the tank and it provide hiding places for the fish, so less stressfull.

the size really depends on the size of your fish. I use 20 gallons but you cannot put a 10'' tang in such tank, so use common sense so that the fish does not lack oxygen.

I use a foam block in an aquaclear HOB filter. I keep a block cycled in my main sump for when it's needed. Liverock works too, but if you have to use meds, which is always a likely maybe, are you willing to sacrifice the rock? If so, then go rock, the fish will probably like it more. The foam block though is disposable or bleachable.
