Default Phosphate

What causes high phosphate? Is it poor water quality to start? Or is more caused by something I'd be doing (IE: too much light, over feeding, what ever)?

Default High Phosphate

Overfeeding is one cause. Infrequent water changes contribute as well. High phosphate promotes hair algae..Yuk
