Flatworm exit by Salifert is supposed to work really well. I have never had flatworms but I did stock up on a couple of treatment kits and I am glad I did because IIRC there were some threads a while ago that said it had been pulled from the retailers by the Government for some reason. Maybe someone close to you has some that they would let you use.

Perform a good water change, vacuum the substrate and cut back on feedings (the remaining worms may starve and die off). That's what I've read.

take all your rock out and dip it in fresh water. Same with all the corals. When you have done this vacuum the sand and do a very big water change. I have had to do this so many times now, and they are a pain in the ass.
The freshwater kills the worms instantly, so if you do it when the lights are off in the tank it should be easier, as they are usually dormant during the dark hours and seem to always hide on the rock or the glass at this time.
