Ideally you remove and try to treat, although the chances of being able to set up a QT for most people in time to treat and NOT kill them from NH3 poisoning is slim. Most people don't have a mature filter sponge or the ability to change 100% of the water daily.
I've done this and lost all the fish in QT. Sometimes you can't win for losing!

However, I did mention at the top that you quarantined before adding, so velvet is much less likely to get into the tank. If you didn't QT beforehand, chances are you don't have the equipment on hand during an outbreak. Really, at that point you already lost.

But, we're talking about ich here, as that is the most common scenario.
