I couldn't find a model number but here is a picture of the ballast showing the wattages. As I said it has two T12 HO and two 250 watt metal halides mogul base. Actually I just checked to original message and I didn't say that. Sorry.

3. Sunlight supply 250 watt HQI pendant and ballast. Comes with brand new Phoenix 12000k bulb (two weeks old) $140
4. Another pendant, also 250 watt HQI with an old odyssea 12000k $30
