IME the first two options are the best. Keep the heat and nitrates down and an increased random flow are a good prevention for RTN. Sometimes it just happens and there is no apparent reason but it is often due to the above mentioned. I unfortunately have a fair bit of experience with both RTN & STN. If you glue the dead area and a bit of the healthy tissue this often tends to stop the necrosis and the healthy tissue will usually encrust over the glue and back down the bare area.
