when you see Ick on the fish it is in its last stage.
It is very difficult to eradicate it all together unless you have no fish in your tank for 45+ days, and can come back with anything you add after unless you QT for 45+ days to make sure it is not present. Not sure what clam you have but it's not killing your hermits. Your hermits are probably killing each other for a bigger homes. Do you have extra shells for them to move into?
