Default Chalice help

My chalice just died and I don't know why? I tested my water with what tests I have
KH 8.5-9
PO4 0-0.01
N03 1

the rest of my corals and fish are just fine, pls help?

I had the same thing happen with mine not sure what happened but I think I had too much light or flow on it. I have a lot more stuff that is a lot harder to take care of so I think it must of been one of the two or maybe both.

whats kind of flow, lighting, ??
where was it placed in the tank?

I find chalices can be a bit sensitive. Have you had it long?

The flow is low to medium
the lighting is a 250W MH
I have had the chalice for five days
and placed it on the ground in a semi shaded place
