Default % water changes & Top off water?

1. Just curious what percentage and how often do you do water changes?

2. Also when running your ATO's or DIY for evaporation do you use salt or fresh?

If you top off your evaporated water with salt water, isn't there a danger of increasing your salinity? Water evaporates, not salt.. or is there a new school of thought that I'm not aware of ?
