Please help me understand why my fish died

I originally posted a thread asking for theories on why my fish might have died. I realize that no one understands my tank and the parameters better than I do. As such, all anyone could actually do is guess and a simple google search will come up with a couple of dozen reasonable guesses. I will try to delete the thread.
