I'm guessing Pinstriped (P. evanidus). I have Kuiter's guide to Fairy and Rainbow wrasses (it actually covers all wrasses - man there are literally 100's of wrasse species ) and the pictures of yours look pretty close to what he has pictured for this species. Here is what he has to say about it:

I see numerous thin longitudinal lines. And FWIW, nothing in the index about "evasive wrasse". So I think that's your ID.
