come with reflector, bulb, ballast, shield, and all the wiring minus a power cord (The test one I have on now doesn't have a ground, probably $5 at home depot). The fixture is designed for two bulbs/ballasts, but there is currently only one in there. I believe the orignal use of this fixture was gym lighting, so you may want to take out the parts and use it as a retrofit kit (this is what I did with the other ballast, bulb). The bulb is about 3-4 months old.
