Around the canister threads? Might need a new O ring, or just lubricate the O ring sparingly.
Connection to canister? Maybe new teflon tape, or not enough or too...

The way I understand the chemical reaction is that the Lanthium chloride combines with both calcium and phosphate to make a new compound that precipitates out and is supposed to be non dissolving...

Sometimes the city is changing a pipe upstream...
Sometimes the city is changing a pipe upstream from you and that sends a lot of gunk into your filters if you happen to be running them at that time.
Hot water tanks pick up this sludge as well....
