Head and lateral line erosion (HLLE). Fresh water dips and copper will not help. Test the water and make sure it's diet is high quality and varied. Make sure it is being fed nori, this often helps.

Unfortunately it sounds like it is declining. Have you tried soaking the mysis in anything? Garlic can sometimes entice a thinning fish. Have you tried brine shrimp? Not a whole lot of nutrition...
